How can physiotherapy help with knee pain?

There are many reasons why a knee may become painful, stiff or difficult to use.

Your physiotherapist will listen to how the problem began, what makes it better or worse and how it is affecting the things you need or want to do.

They will then carry out an appropriate physical assessment. This may include looking at your movement, strength, mobility, balance, functional tasks and the demands you place on your knee.

Depending on your needs, physiotherapy may include:

  • clear advice and education about your symptoms

  • exercises to improve strength, movement and control

  • progressive rehabilitation

  • support with returning to walking, work, exercise or sport

  • guidance on gradually increasing activity or training

  • hands-on treatment where appropriate

  • strategies to help you manage symptoms and build confidence in your knee

The aim is to help you understand your knee, improve what you are able to do and build towards the activities that matter to you.

Knee pain during sport and exercise

Knee pain is common in people who run, lift weights, play field or court sports, cycle or take part in other forms of exercise.

Sometimes symptoms follow a clear injury. In other cases they develop gradually as training, strength, recovery and the demands placed on the knee change.

At Ascent Physiotherapy & Wellness, we understand that returning to sport involves more than simply waiting for pain to settle.

Our physiotherapy experience includes working with recreational athletes as well as athletes in elite and high-performance sporting environments. We use that experience to help people understand the demands of their sport and build rehabilitation towards them in a structured and individual way.

For one person, the goal may be walking comfortably around the park. For another, it may be returning to netball, running, football, the gym or competitive sport.

We deliver the same standard of care for every body.

Knee problems we commonly see

You do not need to know the name of your knee problem before you book.

People come to us with a wide range of symptoms, including:

  • pain around or behind the kneecap

  • pain when walking, using stairs or squatting

  • pain during running or sport

  • tendon-related knee pain

  • stiffness or reduced movement

  • swelling following injury

  • recurring knee pain

  • problems following ligament or meniscal injury

  • knee pain associated with arthritis

  • difficulty returning to activity after a period of rest or injury

Your physiotherapist will assess your individual presentation rather than assuming that everyone with the same diagnosis needs the same treatment.

Knee ligament and sports injuries

Knee injuries can occur during twisting, landing, changing direction, contact sport or sudden changes in movement.

If you have sustained a ligament, meniscal or other sporting injury to your knee, physiotherapy can play an important role in assessing your current function and planning your rehabilitation.

Rehabilitation may include gradually rebuilding strength, movement, balance, confidence and the physical qualities required for your usual activities or sport. Knees and ankles can be prone to recurrent injury when the person returns to their sport or activity a little too soon or a little under-prepared. In our experience, the body responds best to structured and graded exposure to the demands of your sport and/or lifestyle. We follow several specific return to sport protocols that help us prepare your body for your return to full activity.

Where appropriate, we can also work alongside your GP, consultant or other healthcare professionals as part of your recovery. We are happy to follow your medical or surgical doctor's protocols.

Recovery following knee surgery is different for every person.

Your rehabilitation will depend on the type of surgery you have had, your surgeon's guidance, your current level of function and what you would like to return to.

Physiotherapy may involve restoring movement, rebuilding strength and gradually increasing the demands placed on the knee.

As you progress, rehabilitation can become increasingly specific to your everyday activities, work or sport.

We will monitor your progress and adapt your programme as your knee recovers.

Knee Arthritis

A diagnosis of knee arthritis does not mean that you have to stop being active.

In fact, for many people, appropriate exercise and strengthening can form an important part of helping to manage symptoms and maintaining function.

Your physiotherapist can help you explore which activities are appropriate for you, develop a programme that is manageable and gradually build your confidence in using the knee.

The goal is not to chase a perfect x-ray or promise (sadly that is unrealistic) or pray that every arthritis symptom will disappear. The goal is to help you make the best possible use of your knee and continue doing the things that are important to you.

What happens at your first appointment?

Your first appointment gives us time to understand what is happening and how your knee problem is affecting you.

Your physiotherapist will ask about your symptoms, your health, your usual activities and what you would like to return to.

They will then carry out an appropriate physical assessment. This may include examining your knee movement and strength and observing activities such as walking, squatting, stepping or other movements relevant to you.

For people returning to sport, assessment may also consider the physical demands of your chosen activity.

We will explain the findings in clear language and discuss what they may mean for you.

Together, we will agree on a rehabilitation plan that is appropriate for your needs and goals.

Do I need a scan for knee pain?

Not everyone with knee pain needs a scan or an x-ray.

Whether imaging or further investigation is appropriate depends on how your symptoms began, your medical history and the findings from your clinical assessment.

If your physiotherapist feels that further medical assessment or investigation may be appropriate, they will explain why and advise you on the next step.

For many people, a thorough clinical assessment provides a useful starting point for understanding the problem and planning rehabilitation.

When should a knee injury be assessed urgently?

Some knee injuries require more urgent medical assessment.

Seek medical advice if you have sustained a significant injury and are unable to put weight through the leg, if the knee has become markedly swollen or deformed, or if you are seriously unwell alongside a hot, red or swollen joint.

If you are unsure whether your symptoms require urgent medical attention, contact NHS 111 for advice. Call 999 in a medical emergency.
